TOTAL ABFM SEED GRANT APPLIED FOR: $75,000

Seed Grant Received: $50,000 (February 2025)

• Grant Period Per ABFM Agreement: November 14, 2024 through November 14, 2027.

• The balance of the grant ($25,000), will be remitted to FAFP 24 months after the beginning of the period of performance, upon submission of an invoice for this amount and ABFM Foundation review and approval.

EXPECTED OUTCOMES FOR THIS GRANT:

• Grant Reports: FAFP submission of one (1) report annually to the ABFM Foundation by July 30 of each year during the Period of Performance.

• Final Report: A Final Report and presentation will be due at the end of the Period of Performance.

• Program Evaluation: Cooperation with a third-party Program Evaluator who will be under contract with the ABFM Foundation to evaluate the effectiveness of the work of the FAFP Learning Collaborative and share outcomes.

• Engagement with Other Grant Recipients: Actively participate in and engage with other recipients of similar grants and the ABFM Foundation.
