Controlled Substances Model Consent Form
As the result of the enactment of HB 7095, effective January 1, 2012, each and every patient who receives ANY controlled substances for the treatment of chronic non-malignant pain in an office setting (even if such treatment is not provided in a "pain-clinic") MUST enter into an agreement with the treating physician. This agreement must meet strict standards, including agreements that only one physician and one pharmacy will be used, as well as other protections designed to prevent the likelihood of abuse.

FAFP has developed a generic, Model Consent Form for use in your office. FAFP members are encouraged to review this form, along with any patient/physician agreements, with legal counsel to ensure sufficient proper compliance for your practice.
